RWJBarnabas Health, Primary Care Service Live is seeking a Full-Time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to be part of the primary care service line providing coverage to southern NJ.

Qualifications

• CDS and DEABLS
• Master's degree required or Graduate Level Certification
• APNs
• Nurse Practitioner Certification
• RN License
• PAs
• PA license
• NCCPA

Essential Functions

• Performs comprehensive initial and episodic health assessments;
• Initiates and implements an individualized nursing and medical plan of care in collaboration with members of interdisciplinary team;
• Initiates appropriate diagnostic, therapeutic, medical and nursing actions in conjunction with physician;
• Monitors and adjusts patients medication according to protocol or medical plan of care;
• Implements nutritional therapies, as appropriate;
• Triages patient problem calls which requires medical attention;
• Orders appropriate laboratory and diagnostic procedures;
• Recognizes normal and abnormal laboratory findings and the pathology involved;
• Documents information in patients charts following documentation standard;
• Provides health education to the patient and family (e.g., management of side effects, anticipatory teaching);
• Manages medical emergencies as indicated, and according to standing orders;
• Provides monitoring and continuity of care between health care settings;
• Provides patient and family counseling and reassurance in regard to psychological and or psychosocial issues;
• Initiates proper referrals to private or community agencies;
• Acts as a resource and teacher for other members of the health care team;
• Shares knowledge and expertise with staff;
• Collaborates with the nursing and medical staff at partner and affiliate sites to coordinate continuity of care;
• Evaluates the quality of care as well as the quality of life for patients through assessment of patient response and recommend interventions, as indicated.
