Advanced Practice Provider - APRN or PA - Heart & Vascular

About the Role

Advanced Practice Provider (APP) is a provider of heart & vascular health care services who synthesizes and utilizes evidence-based practice, research, and current clinical knowledge. Working in collaboration with a multidisciplinary health care team, the APP demonstrates an advanced level of medical and/or nursing knowledge, clinical and technical competence, sound clinical judgment, professionalism in interpersonal and general communication skills, timely and complete documentation, and responsibility for ongoing professional development and competency validation.

The APP role, in partnership with supervising/collaborating physician(s), includes history taking, clinical assessment, physical examination, order entry, interpretation of diagnostic studies, clinical diagnosis, treatment, and evaluation of care for the given population. The APP may have On Call duties as assigned by practice and will participate in inpatient weekend rotation. Performs other duties as assigned.

Responsibilities

• Complete Quality Assurance chart reviews (25 charts for new grad, 10 charts for an established provider) and keep records at practice location.
• Evaluate signs and symptoms, including age-appropriate changes, noting pertinent positives and negatives.
• Assess complex acute, critical, and chronically ill patients; obtain relevant comprehensive and problem-focused health histories, prioritizing data collection and using collateral sources as necessary.
• Develop and reformulate differential diagnoses by priority, identify comorbidities, and use diagnostic strategies and technical skills to monitor and sustain physiological function and ensure patient safety.
• Participate in daily team transition huddles to facilitate effective coordinated patient care.
• Utilize evidence-based practice, prescribe appropriate pharmacologic (including medication reconciliation) and non-pharmacologic treatment modalities, implement treatment and referral orders, and interpret and evaluate lab and diagnostic testing.
• Monitor and ensure the quality of healthcare practice through consultation, collaboration, continuing education, certification, and evaluation.
• Adapt teaching and learning approaches based on physiological and psychological changes, age, developmental stage, cognitive status, readiness to learn, health literacy, environment, and available resources.
• Work collaboratively to identify processes and principles to meet quality core metrics and documentation standards.
• Demonstrate clinical reasoning, build collaborative intra- and interprofessional relationships, and advocate for patient populations and the profession through active participation in health policy.
• Address the development and implementation of system policies affecting services.

Requirements

• Master’s degree with an Advanced Practice Nursing Focus and National Board Certification required for APRN.
• Master’s degree with appropriate National Certification required for PA.

Qualifications

• APRN: National Board Certification as a CNP, CNS, CNM, or CRNA (certification to match scope of practice).
• PA: National Commission on Certification of Physician Assistants (NCCPA).
• Basic Life Support (BLS) certification required.
• Advanced Cardiovascular Life Support (ACLS) certification required.
• Current Ohio Board of Nursing license for APRN.
• Current Ohio License 50RX – License to practice as a physician assistant with prescriptive authority.
• Maintain collaborative/supervision agreement as required through the Ohio Medical Board/Ohio Board of Nursing, including yearly review/Quality Assurance and evaluations.
