About the role

The Advanced Practice Professional (APP) has coordination and clinical skills required for the comprehensive management of critical care medicine patients, including biological, social, and psychological assessments. They will possess specialized skills in the care of patients and families undergoing operative and other invasive procedures. The APP coordinates the involvement of medical/surgical specialty teams necessary to develop a comprehensive plan for the patient and reviews the care plan with the other members of the clinical team, participates in directing that care. The provider will work with the team to ensure adherence to protocol requirements and regulatory guidelines. They will perform diagnostic/therapeutic procedures, peri-procedural services, and coordinates discharge planning to next level of care (as applicable).

Responsibilities

  • Manages comprehensive and individualized care for patients and families in the inpatient/critical care phase of hospitalization and immediate post transfer to stepdown, ward or discharge.
  • Completes daily rounds; consults and/or pre-procedure/evaluations.
  • Completes integrated assessments.
  • Orders additional tests or consults as deemed appropriate with the collaboration of critical care faculty or by following established clinical Guidelines and Protocols.
  • Communicates results of diagnostic tests following established clinical guidelines and scope of practice.
  • Consults and communicates with clinic and hospital staff regarding abnormal findings in the evaluations of patients.
  • Facilitates patient discharge or transfer by coordinating appropriate services and tests for follow-up (as applicable).
  • Develops a comprehensive individualized plan of care for critical care patients, which includes teaching, discharge or transfer planning and necessary referral to other members of the health care team as necessary.
  • Evaluates critical care patients and serves as a coordinator for follow up procedures and tests in the ambulatory setting (as applicable).
  • Assists in the collection of outcome measures (as applicable).
  • Demonstrates expertise in caring for patients across the age continuum of their educational experience and certification.
  • Identifies unstable or complex critical care patient care situations and facilitates appropriate intervention in the hospital setting.
  • Recognizes emergency situations and initiates effective emergency care.
  • Documents the plan of care, interventions, and evaluations in a clear and concise manner for every patient visit in the patient’s medical record following approved hospital/UHA policies and compliance standards for documentation.
  • Collaborates with other leaders and staff to effectively achieve patient outcome-based improvements in care.
  • Serves as a resource person for staff caring for the critical care patient.
  • Provides educational programs to professional and paraprofessional personnel in the area of expertise.
  • Participates and initiates inter-disciplinary approaches to patient care, program development and education.
  • Serves as a professional role model.
  • Participates and provides leadership for committees and task forces.
  • Reviews and interprets critical care data from sources such as patient registries, clinical and financial reports, length of stay, access to care, and cost per case reports and recommends/implements changes as necessary.
  • Participates in clinical trials and research studies with the Principal Investigator.
  • Assists critical care Faculty, Administrators and Directors in the collection of outcome measures.
  • Other duties/projects as assigned.

Qualifications

  • Minimum Qualifications for Nurse Practitioner: Graduate from a CCNE accredited nursing program with Master’s Degree in Nursing. National Certification by one of the recognized APRN Certification Centers. Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC). Advanced Practice Registered Nurse License issued by West Virginia Board of Nursing. American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the department.
  • PREFERRED QUALIFICATIONS for Nurse Practitioner: Acute care or critical care experience preferred. ACNPs preferred but will consider FNPs.
  • Minimum Qualifications for Physician Assistant: Bachelor’s Degree as a Medical/Surgical (as applicable) Physician Assistant required. Graduate of an ARC-PA approved Physician Assistant (PA) training program required. Certification by the National Commission for Certification of Physician Assistants required. Must be licensed or eligible for licensure by the WV Board of Medicine or WV Board of Osteopathic Medicine. American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the department.
  • PREFERRED QUALIFICATIONS for Physician Assistant: Master’s Degree as Physician Assistant preferred. Critical care or critical care experience preferred.
