Advanced Practice Clinician

About the Role

Provides clinical leadership to enhance compliance with quality and care standards. Manages service delivery of inter-professional teams and offers advanced nursing care in accordance with regulations and standards. Acts as a key resource for clinical and operational guidance, supporting team outcomes. Works under supervision of the Clinical Director for the Nurse Practitioner Program.

Responsibilities

• Provides full scope of advanced nursing practice, including patient assessment, care planning, interventions, and education.
• Evaluates patient responses, revises care plans, and collaborates with physicians to develop protocols.
• Ensures care quality aligns with standards like Patient-Centered Medical Home, HEDIS, and QARR.
• Coordinates with patients, families, and healthcare teams for comprehensive care management.

Requirements

• Licensed and registered as a Nurse Practitioner in New York State, with relevant specialty certification.
• Master's degree in Nursing from an accredited program; current certification from ANCC or AANP.
• Minimum two years of full-scope nurse practitioner experience; clinical home care or managerial experience preferred.
• Knowledge of quality measures, coding, and bilingual skills as needed.
• For psychiatric roles, specific experience and certification required.
