Advance Practice Primary Care Provider

About the role

We’re hiring an Advanced Practice Provider (NP/PA) for our Direct Primary Care practice. This position offers autonomy, a manageable patient load, and competitive benefits.

Responsibilities

• Provide primary care services to a dedicated panel of patients
• Conduct physical exams, order and interpret diagnostic tests, and develop treatment plans
• Establish and maintain long-term patient relationships
• Offer health promotion, disease prevention, and education
• Work collaboratively with other providers in a supportive, team-oriented environment
• PRACTICE EVIDENCE-BASED MEDICINE WITH A FOCUS ON HOLISTIC AND PREVENTATIVE CARE
• WORK COLLABORATIVELY WITH THE MARKETING TEAM TO GROW OUR PRACTICE AND SERVE MORE PATIENTS

Requirements

• Current Nurse Practitioner (NP) or Physician Assistant (PA) licensure
• Experience in primary care or family medicine preferred
• Passion for partnering with your patients and patient-centered care
• Ability to build trust and strong relationships with patients
• Excellent communication and clinical skills
• Independent, self-motivated, and proactive approach to care
• Ability to self-schedule and to prioritize patient needs
• Home health or hospice experience is encouraged
