Advance Practice Nurse
About the role
The Advanced Practice Nurse will conduct visits to patients in accordance with local, state, and federal regulations. They will communicate visit findings to designated members of the interdisciplinary team, facilitate access to appropriate support models of care, consult with other members of the interdisciplinary team to contribute to the patient's plan of care, prepare and maintain accurate patient records, charts, and documents, assist in developing clinical practice guidelines/standards in support of quality care as needed, and ensure collaboration with patients, caregivers, medical directors, and members of the interdisciplinary team.
Responsibilities
- Conduct visits to patients in accordance with local, state and federal regulations.
- Communicate visit findings to designated members of the interdisciplinary team.
- Facilitate access to appropriate support models of care (i.e., rehabilitation, on-going aggressive treatment).
- Consult with other members of the interdisciplinary team to contribute to the patient's plan of care.
- Prepare and maintain accurate patient records, charts and documents.
- Aid in the development of clinical practice guidelines/standards in support of quality care as needed.
- Ensure collaboration with patients, caregivers, medical directors, and members of the interdisciplinary team.
Requirements
- Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
- Nurse Practitioner's license in the state of operations.
- 1+ years' experience working within Family Practice, General Practice, Geriatric care or related field.
- Valid driver's license and a reliable source of transportation.
- Willingness to travel.
- Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required.
- Commitment to ongoing professional development and clinical excellence.
Qualifications
- Education: Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
Skills

Pay
$125,000 to $135,000 per year.
Schedule
Part-time (22.5+ hours/week) and Full-time positions available.
