About the Role
The APS Case Worker, under the direct supervision of the Adult Protective Supervisor/Assistant Supervisor, responds to intakes, referrals, and crisis situations. This position assesses reports of abuse, neglect, or exploitation, conducts casework, follows up on substantiated cases, and completes documentation in accordance with Illinois Department on Aging (IDoA) guidelines.
Responsibilities
- Conduct home visits to assess clients' needs and assist clients in accessing services.
- Complete investigation assessments for adult abuse and exploitation, collect evidence, and interview alleged victims, abusers, and collaterals.
- Determine the need for emergency intervention services, provide crisis intervention, and participate in 24/7 on-call responses.
- Seek and coordinate assistance from other providers on behalf of clients.
- Meet all required response timeframes for investigations, casework, follow-up, at-risk assessments, and case closures as established by State Law and IDoA policies.
- Consult with supervisor to determine case substantiation (substantiated, unsubstantiated, or unable to be substantiated).
- Develop a case plan in consultation with the client and supervisor.
- Provide services as needed, including orders of protection, guardianships, legal interventions, emergency safe shelter, after-hour services, and EIS.
- Accurately complete and process referrals for clients to other agencies and private individuals.
- Complete all required documentation and submit files to IDoA as mandated; prepare VOA reports.
- Provide monitoring, follow-up, and risk assessment for each client on the assigned caseload.
- Prepare and present cases for M-TEAM meetings and incorporate recommendations into client case plans.
- Collaborate with staff, other agencies, and private individuals to locate hard-to-reach clients.
- Maintain timely and accurate documentation of services provided to clients.
- Work closely with other casework and case management staff.
- Participate in activities and events to promote agency services, including giving presentations on benefits and services.
