About the Role

Nurse Practitioner responsible for in-home post-discharge care management in the Oak Ridge area.

Responsibilities

• Conduct in-home patient visits to assess clinical status and deliver post-discharge medical care.
• Review and interpret discharge summaries and progress notes.
• Collaborate with home health professionals and care team members for seamless care coordination.
• Provide medical consultations, prescribe medications as appropriate, and oversee ongoing care plans to support recovery and reduce readmissions.

Requirements

• Valid Tennessee medical license and active Medicare provider number.
• Certified Nurse Practitioner in good standing.
• Ability to travel within a 35-mile radius to see patients.
