Adult Day Program Assistant

Ebenezer is seeking an Adult Day Program Assistant to join our team in Bloomington, MN. This role encourages socialization and assists in planning and implementing programming needs for individual participants and groups, as well as personal care duties. Responsible for ongoing charting and case management within our Skilled Nursing Facility.

Martin Luther Care Center, part of the Martin Luther Senior Living Campus, is located in East Bloomington near woodlands/wetlands and along the Minnesota River Bluffs. The campus offers 137 units across Skilled Nursing, Memory Care, and Transitional Care. Responsibilities

• Assist the Director in the efficient operation of the program:
  • Conduct tours.
  • Meet with families and assist in filling out enrollment paperwork.
  • Communicate with Case Managers and families as needed.
  • Oversee charting of Program Assistants to ensure accuracy and timeliness.
  • Assist with setting up care plans.
  • Conduct quarterly/annual assessments as needed/directed.
  • Assist with office duties (attendance, billing).
  • Function as Lead staff in absence of Director.
• Responsible for individual case management:
  • Set up participants’ charts per licensing requirements.
  • Ensure all forms are completed by family and staff and updated as necessary.
  • Maintain monthly charting, quarterly care plans, and health updates.
  • Document monthly health education, fire drills, blood pressures, and weights for each participant.
• Provide direct care for Adult Day Program participants:
  • Prepare, implement, and clean up daily programs (e.g., crafts, woodworking, games, music).
  • Provide snacks and noon meals to participants, including setup, feeding assistance, and cleanup.
  • Assist staff in developing the monthly calendar.
  • Handle daily menu planning and ordering of meals and snack supplies.
  • Perform routine personal care and grooming (including baths).
  • Assist identified participants with all toileting needs.
  • Promote and encourage individual care.
  • Provide individual and family counseling for participants as needed.
  • Communicate participants' needs and individual plan of care to staff.
  • Observe, assess, and intervene with participants' health concerns.
  • Report any problems, progress, and observations to the Director.

Requirements

• CPR certification and Basic First Aid Training.

Qualifications

• Preferred: A.A./A.S. or less than 1 year of experience.

Schedule

• 80 hours per two weeks (full-time).
• Day shift: 8:00 AM – 4:30 PM.
• No weekends.

Pay

An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical. If eligible, a sign-on bonus may be offered based on the approved program at the time of the offer.
