Waypoint offers comprehensive services, including a special purpose private school for children with autism and other developmental disabilities, case management and outpatient therapy services for adults and children, and residential and in-home supports for adults. Waypoint has expanded to the Western Region of Maine, formerly known as The Progress Center, and our department is growing.

About the Role

The Case Manager assists individuals and their teams to identify needs and develop a plan of care, initiate referrals, and monitor services while working closely with the multi-disciplinary team. The Case Manager coordinates and advocates for services identified in the plan, monitors progress, and evaluates the effectiveness of services. Supervision and support to achieve and maintain social work licensure (LSW) is also available. This is a hybrid remote position after successful onboarding, based out of our Oxford, Maine home office for Adult Case Management.

Case Managers provide support to adults with intellectual disabilities/autism and their families, serving as an advocate, ensuring choice and satisfaction with services, and monitoring the individual's health and safety.

Responsibilities

  • Assist individuals and their teams to identify needs and develop a plan of care.
  • Initiate referrals and monitor services.
  • Coordinate and advocate for services identified in the plan.
  • Monitor progress and evaluate the effectiveness of services.
  • Provide support to adults with intellectual disabilities/autism and their families.
  • Serve as an advocate, ensuring choice and satisfaction with services.
  • Monitor the individual's health and safety.

Qualifications

  • BA/BS in human services or closely related field or BA/BS in unrelated field + minimum 1 year experience in adult IDD services.
  • Knowledge of the system of care in Maine preferred.

Requirements

  • Reliable vehicle with proper insurance.
  • Valid driver's license issued by state of residence with clean driving record.
  • Proficient in speaking, reading, and writing English with strong interpersonal and communication skills.
  • Able to complete agency and state-mandated training programs with proficiency.

Pay

From $23.00 per hour
