Responsibilities
- Coordinate admissions of home care clients through development, implementation, and evaluation of a plan of care, working in collaboration with the interdisciplinary team.
- Absorb and document patient conditions and family/caregiver conditions accurately and completely during each visit.
- Complete required patient and Agency paperwork in an accurate, thorough, legible, and timely manner.
- Work cooperatively with patients, care providers, family members, and Agency staff.
- Communicate patient issues and concerns to the Case Manager and / or Clinical Manager in a timely manner.
- Provide direct assistance with activities of personal care, maintenance of patient health, Agency facilitation of treatment for patients, and contribute to a health environment for patients.
- Support patient care by reviewing care plan in the home prior to providing cares.
- Notify Case Manager with issues/concerns regarding care plan and when cares need revision.
- Provide assistance with daily living under the supervision of the Case Manager.
- Explain to patient the care being provided and involve patient in cares.
- Complete mobile care upon arrival and departure from patients' home per Agency policy.
- Complete mileage logs timely and accurately.
- Work cooperatively with patients, care providers, and family members, as well as with appropriate Agency staff.
Requirements
- High school diploma or equivalent.
- Current certification as a Certified Nursing Assistant (CNA).
- Experience in a healthcare setting preferred.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
