Key Responsibilities

• Cook up the admission process for new residents, including assessments and documentation.
• Conduct resident assessments and collaborate with interdisciplinary teams to develop care plans.
• Ensure all admissions meet facility policies, state, and federal regulations.
• Provide education to families and residents regarding services and procedures.
• Act as a liaison between families, medical staff, and the administrative team to ensure smooth transitions.
• Maintain accurate and up-to-date resident records in compliance with regulatory standards.

Qualifications

• A current and active Registered Nurse (RN) license in New York State.
• Previous experience in admissions, case management, or a similar nursing role preferred.
• Strong organizational, communication, and interpersonal skills.
• Ability to work independently, manage multiple priorities, and ensure excellent patient care.
• Compassionate, team-oriented, and able to build strong relationships with residents and families.
