Responsibilities

• Coordinate all aspects of patient admission, ensuring a smooth and timely process from intake to program entry.
• Educate patients' families and referral sources about hospice services and benefits.
• Gather initial intake information, complete admissions paperwork, and confirm benefits with referral centers.
• Conduct clinical evaluations and serve as a resource for marketing staff.
• Follow up with referral sources to ensure admission processes are implemented efficiently.

Qualifications & Skills

• Licensed Registered Nurse in the applicable state.
• Knowledge of high-quality hospice care and ability to provide patient care.
• Strong communication, organizational, and interpersonal skills.
• Outgoing, perceptive, self-motivated, and able to work harmoniously with diverse groups.
• Prior management experience preferred; capable of functioning effectively under stress.
