About the role
Frontier Home Health is searching for an Admissions Nurse in Colorado Springs. We are a recognized top home healthcare agency in Pueblo and are expanding our services in Colorado Springs. The Admissions RN plays a key role in this expansion.
Responsibilities
- Complete Start of Care (SOC), admission, and resumption of care assessments, including comprehensive physical, functional, and psychosocial evaluations.
- Develop and implement individualized plans of care in accordance with physician orders, patient needs, and Medicare guidelines.
- Perform accurate and timely OASIS assessments and clinical documentation in compliance with regulatory and agency standards.
- Collaborate with physicians, therapists, and interdisciplinary team members to coordinate patient care.
- Communicate changes in patient condition and obtain/verify physician orders as needed.
- Administer skilled nursing care, medications, and treatments as prescribed.
- Identify needs for additional disciplines, durable medical equipment (DME), and community resources.
- Initiate and support discharge planning and care transitions from admission through episode completion.
- Ensure all documentation supports quality outcomes, compliance, and reimbursement requirements.
- Participate in case conferences, care coordination, and quality improvement initiatives (QAPI).
Requirements
- Active, unrestricted Registered Nurse (RN) license in the state of Colorado; graduate of an accredited nursing program (BSN preferred).
- 2+ years of RN experience preferred; home health and OASIS experience strongly preferred.
- Current CPR certification required.
- Valid driver’s license, reliable transportation, and auto insurance.
- Knowledge of home health regulations, OASIS, and nursing assessment/care planning.
- Ability to maintain HIPAA compliance and adhere to all regulatory and agency standards.
- Completion of required TB screening and health documentation.
