About the Role

As an Admissions Registered Nurse (RN) in the St. Louis area, you’ll assess and support patients and families during a critical time, ensuring exceptional care and coordination. This full-time, on-site role covers St. Louis County, St. Charles County, and surrounding areas, with occasional travel to Franklin and Jefferson counties.

Responsibilities

  • Assess patient and family needs—physical, psychological, social, and spiritual—and collaborate with physicians, the Interdisciplinary Group (IDG), and families to develop and implement care plans.
  • Maintain communication with attending physicians, the Medical Director, IDG, Team Director (TD), and external agencies to optimize care and resource use.
  • Document patient records thoroughly, ensuring plans of care reflect baseline, current, and evolving needs, including interventions, actions, and goals.
  • Participate in on-call or admissions rotations as assigned.
  • Provide education to patients and caregivers based on assessed needs, condition, and the care plan.
  • Address unusual or complex patient/family issues with professionalism and care.

Requirements

  • Active RN license in the state(s) of practice; associate degree preferred.
  • CPR certification (required for direct patient care in Georgia).
  • Previous nursing experience in hospice, home health, medical-surgical, or population-specific care (e.g., geriatrics, pediatrics).
  • Willingness to serve on-call and participate in admissions rotations.
  • Reliable vehicle, valid driver’s license, and minimum required auto liability insurance.
  • Strong communication, organization, and teamwork skills.
  • Proficiency in Microsoft Office Suite and EMR systems.
  • Physical ability to lift patients over 200 pounds (with assistance), stand, walk, sit, stoop, and perform other movements frequently.
  • Comfort working in indoor environments with exposure to vibration and moderate noise.

Pay

$80,000 – $90,000 per year, plus mileage reimbursement.

Schedule

Tuesday–Friday, 10:00 AM–8:00 PM, with a weekend rotation.
