About the role
Pioneers in the home care industry, Interim HealthCare® is looking for a Home Health Admission RN with a passion to provide exceptional patient care while enjoying the flexibility and autonomy that comes with it.
Responsibilities
- Complete initial and ongoing comprehensive assessments.
- Cookordination for patient/client services.
- Provide care according to the patient’s/client’s plan of care/service plan.
- Participate with the patient/client in clarifying mutually agreed upon goals and incorporate these goals into the plan of care/service plan.
- Conduct home visits as assigned, assessing the patient’s condition, progress toward goals, providing patient care, teaching and other skilled interventions as ordered.
- Develop a care plan in collaboration with Case Manager, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions, including the patient and family in the planning process.
Requirements
- Graduate of an accredited nursing program and active RN license in Colorado.
- Minimum of one (1) year of nursing experience.
- Recent Home Health and Oasis experience required.
- CPR Certification (demonstration course required).
Qualifications
- Knowledge of state, federal, and licensure regulations.
Skills
- Able to work independently.
- Effective communication to patient/family, team members, and other health care professionals.

Pay
$90,000 per year
Schedule
Monday-Friday, 8a-5p, with on-call rotation
