Admissions Registered Nurse (RN)
The Role
The Admissions Nurse will be responsible for completing an initial assessment of patients and families to determine home care needs, outlining referrals and admissions, articulating hospice philosophy, coordinating care plans, and marketing to acute hospitals.
Requirements
- Associate's Degree in Nursing required, Bachelor's Degree preferred
- 2-4 years of related Hospice experience
- Licensed as a Registered Nurse within the state of operations
Qualifications
- Ability to communicate effectively with other members of the clinical team
- Knowledge of hospice philosophy and goals
- Experience marketing to acute hospitals
