Admissions Registered Nurse
Hospice Philosophy
The Admissions Nurse will articulate the hospice philosophy, goals, and objectives.
Responsibilities
- Completes an initial assessment of patient and family to determine home care needs and identifies problems for each patient upon admission.
- Ability to outline the patient/family referral and admission.
- Uses assessment data to determine care plan needs and initiates the plan of care.
- Communicates with other members of the clinical team to coordinate the care plan.
- Marks up to 50% of time to acute hospitals in the area.
Qualifications
- Associate's Degree in Nursing required, Bachelor's Degree preferred.
- 2-4 years of related Hospice experience.
- Licensed as a Registered Nurse within the state of operations.
