Pay

$43.50 - $48.05/hr. Placement within the wage range for this position is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.

Schedule

Full time: Tuesday - Sunday 12:00 pm - 8:30 pm

Responsibilities

  • Complete admission assessments for incoming residents, including physical and clinical evaluations.
  • Coordinate admissions to ensure a smooth transition into the facility.
  • Review hospital records, physician orders, medications, and treatment plans prior to admission.
  • Collaborate with physicians and interdisciplinary team members to support continuity of care.
  • Ensure accurate and timely completion of admission documentation, authorizations, and medical records.
  • Educate residents and families regarding admission procedures, care plans, and facility policies.
  • Assess residents for appropriate level of care and identify immediate clinical or safety concerns.
  • Coordinate room assignments, equipment, and resident care needs upon admission.
  • Maintain compliance with state, federal, facility, and HIPAA regulations.
  • Assist with resident transfers and discharge coordination as needed.

Qualifications

  • Preferably 1-3 years of clinical nursing experience with at least two years in long term care and two years of successful experience in administration, consultation or involvement in the survey process.
  • Must have an LPN license in good standing.
