Altus Hospice is seeking a Full Time Compassionate Resource Nurse (RN) to provide patient care in and around the North Houston Area. This role works with the intake team, referring physicians, and other referral sources to promote the hospice philosophy of care.

About the role

Our Resource Nurses (RN) are responsible for assuring that the Hospice programs are effectively represented to patients, families, and others who have an interest in hospice through pre-assessments, information sharing, and/or admissions in a patient's home or in a healthcare facility.

Schedule

Monday - Friday, 10 am - 6:30 pm
Territory: North Houston

Responsibilities

  • Reviews clinical referral information and responds to requests and inquiries as appropriate.
  • Collaborates with Clinical Director regarding the eligibility and appropriateness of clients for hospice services.
  • Provides information, pre-assessment, and admission services to patients who have been referred to the hospice; conducts pre-assessment visits in the home or healthcare facility as necessary.
  • Communicates with the Intake Department, the referring physician, and the Hospice Medical Director to assure appropriateness of each referral and that the necessary certifications are obtained.
  • Applies nursing principles and techniques in the treatment of patients.
  • Provides nursing care, emotional support, and education to patients and families.
  • Encourages patients and family members to participate in the patient's plan of care to meet established goals.
  • Participates in On-Call duties as assigned and defined by Clinical Director and Program Director.

Requirements

  • Bachelor's degree in nursing or healthcare preferred.
  • Current unencumbered license as a Registered Nurse.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
  • Hospice/home health experience preferred but not required.
  • Computer proficiency in typing and various programs, including experience with EMR.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to dependable transportation.

Pay

$76,000 - $87,000 per year (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position.
