Responsibilities

• Determines a patient’s eligibility for hospice after receiving a physician referral.
• Works closely with the account representative at hospice to screen for insurance benefits.
• Works directly with the case manager and social worker following the patient’s admission to the hospice program.
• Performs initial assessment in the absence of the case manager using age-specific physical, psychological, and social assessments to maximize the comfort and health of terminally ill patients and their families.
• Participates in the performance improvement program.
• Deals with pressure to meet deadlines, maintains accuracy, and handles constantly changing situations.
• Deals with a variety of people, handles stressful situations, and handles conflict.

Requirements

• Current state registered nurse license required.
• Graduate of an accredited school of nursing.
• Experience as an RN commensurate with one of the following preferred:
• One-year acute care or long-term care experience within the last two years;
• One-year home health care experience within the last two years;
• One-year community health or public health experience within the last two years;
• One-year hospice experience within the last two years.
