About the Role

Hiram W. Davis Medical Center, located in Petersburg, Virginia, is a medical facility providing acute medical diagnoses, skilled care, intermediate care, and long-term care to individuals receiving services in DBHDS facilities and within the community that have intellectual and/or developmental disabilities. Although HWDMC provides long-term care services, current admissions are being accepted for temporary stays and special hospitalization only. Reasons for special hospitalization include but are not limited to acute care, step-down from a community hospital to stabilize, and rehabilitation.

The Admissions & Discharge Coordinator – LPN carries out the screening process for admissions and coordinates the discharge process for individuals with intellectual or developmental disabilities discharging to community placements. This position is highly interactive and acts as a critical link between various entities (both inside and outside the organization), working with all interdisciplinary team members including physicians, nurses, social workers, therapists, dietitians, MDS Coordinators, and Pharmacists. This role requires effective communication with DBHDS facilities, Central Office, families, Community Service Boards (CSB’s), and private providers, and plays an integral role in facilitating discussion in team conferences.

The hours of this position are Monday through Friday, typical business hours, but may vary based on the needs of the facility.

Responsibilities

  • Collects information to assist in the decision of appropriateness of admission.
  • Screens admissions for complications beyond the capacity of HWDMC.
  • Coordinates and leads the discharge process for individuals with ID/DD discharging to community placements, including the individual, HWDMC team members, CSB, AR/LG, residential provider, CIM, and other community providers as appropriate.
  • May require limited travel to the community including group homes or other facilities.
  • Manages paperwork related to discharge planning, training, visitation, and post-move monitoring documents as well as prescriptions, orders, and letters of need.

Requirements

  • A valid Virginia LPN license.
  • Valid driver's license.
  • Computer skills, including comfort with using Microsoft Office Suite.
  • Close attention to detail and good organizational skills.

Qualifications

  • Experience in Long Term Care and/or Admissions & Discharge Coordinator experience.
  • Excellent oral and written communication skills.
  • Positive attitude, friendly and comforting demeanor, and ability to interact with others with ease.
  • Proven ability to work interactively with all clinical disciplines and establish effective interpersonal relationships with staff.

Pay

$55,439 - $76,505
