Admission Coordinator I

Role Overview: The Admissions Coordinator I is responsible for facilitating patient referrals and supporting the admission process for hospice and palliative care services. They serve as the first point of contact, providing compassionate, clear information to patients, families, and healthcare partners, and coordinating assessments to ensure timely and appropriate care access. Key Responsibilities Respond to inquiries and provide information about care options, building relationships with referral sources and healthcare providers.Accurately document referrals, schedule assessments, verify insurance, and assist with admission coordination.Identify and communicate barriers to admission, supporting timely resolution.Collaborate with clinical teams, physicians, and facilities to ensure seamless patient access and care continuity.Represent the organization professionally, conducting tours and providing service with compassion and sensitivity. Qualifications & Skills High school diploma or equivalent; healthcare, business, or related degree preferred.Minimum six months of clerical or customer service experience; healthcare experience desirable.Strong communication, listening, and interpersonal skills; ability to handle emotionally sensitive situations.Proficiency with computer systems, EMR, and data entry.Ability to manage multiple priorities with accuracy and professionalism.Understanding of hospice philosophy, insurance, and healthcare processes is advantageous.
