Responsibilities

  • Receive and screen all incoming referral calls for the TLC program.
  • Coordinate and track referrals from providers, community agencies.
  • Assist with assigning cases to clinicians and ensuring a smooth coordination.
  • Enter referral information in the Electric Health Record.
  • Maintain referral databases and provide statistical data to supervisors and grantors.
  • Auxiliary with day-to-day operations of the TLC program: creating training and policy binders and files, article binders, and ordering supplies and materials.
  • Auxiliary with grant requirements and duties.
  • Regularly attend team meetings and take minutes, coordinate meetings, coordinate requests on behalf of the clients.
  • Manage requests for records and documentation from outside agencies.
  • Maintain records to conform to agency policies and procedures and to regulatory agencies directives.
  • Auxiliary with special projects or ad-hoc tasks and run reports.
  • Copying, filing, reporting and maintaining records.

Requirements

  • Associates degree or 5 years of experience as an Administrative Assistant.
  • Excellent organizational skills and detailed oriented.
  • Exceptional customer services abilities.
  • Excellent computer skills, including Microsoft Excel and Word.
  • Strong writing skills with ability to compose and edit correspondence.
  • Ability to comprehend and handle the varied activities of a large and diversified Agency.
  • Strong interpersonal skills with the ability to establish and maintain effective working relationships and to relate well with supervisors, program officials, employees and community partners.
  • Good knowledge of community resources and their use.
  • Ability to communicate effectively in English, both orally and in writing.
  • Bi-lingual skills a plus.
