Responsibilities
- Receive and screen all incoming referral calls for the TLC program.
- Coordinate and track referrals from providers, community agencies.
- Assist with assigning cases to clinicians and ensuring a smooth coordination.
- Enter referral information in the Electric Health Record.
- Maintain referral databases and provide statistical data to supervisors and grantors.
- Auxiliary with day-to-day operations of the TLC program: creating training and policy binders and files, article binders, and ordering supplies and materials.
- Auxiliary with grant requirements and duties.
- Regularly attend team meetings and take minutes, coordinate meetings, coordinate requests on behalf of the clients.
- Manage requests for records and documentation from outside agencies.
- Maintain records to conform to agency policies and procedures and to regulatory agencies directives.
- Auxiliary with special projects or ad-hoc tasks and run reports.
- Copying, filing, reporting and maintaining records.
Requirements
- Associates degree or 5 years of experience as an Administrative Assistant.
- Excellent organizational skills and detailed oriented.
- Exceptional customer services abilities.
- Excellent computer skills, including Microsoft Excel and Word.
- Strong writing skills with ability to compose and edit correspondence.
- Ability to comprehend and handle the varied activities of a large and diversified Agency.
- Strong interpersonal skills with the ability to establish and maintain effective working relationships and to relate well with supervisors, program officials, employees and community partners.
- Good knowledge of community resources and their use.
- Ability to communicate effectively in English, both orally and in writing.
- Bi-lingual skills a plus.
