ESSENTIAL JOB FUNCTIONS

  • Establishes rapport and positive working relationships with clients while maintaining professional ethics and boundaries.
  • Enhances a client’s daily living skills by assessing and providing various modalities of teaching and assistance (e.g. modeling, prompting, hands-on assistance).
  • Develops, implements, and monitors person-centered plans with the client in all aspects of care, up to and including the intake and discharge processes.
  • Captures referrals, obtains and maintains entitlements, manages money, accesses treatment, medical care, transportation, housing, wellness/nutrition, legal services, education and employment, etc.
  • Aids clients in identifying potential barriers to achieving their aspirations and goals, and links them to appropriate services to address these obstacles.
  • Mets regularly with each client to review progress, mental health stability, upcoming appointments and activities.
  • Supports clients to learn local transportation while providing transportation to meet client needs.
  • Develops and maintains working relationships with families, natural supports, and external providers and advocates on behalf of clients as necessary.
  • Facilitates/co-facilitates psychoeducational groups as indicated.
  • Completes service entry documentation in accordance with contract and agency requirements.
  • As applicable, administers breathalyzer tests and urinalysis samples.
  • Conducts rounds within program, as indicated.
  • Attends and participates in staff and AllTreater’s meetings, rounds, and supervision.

QUALIFICATIONS

  • Bachelor's degree in a related field and one (1) year of relevant experience; OR a high school diploma or equivalent and three (3) years of human services experience, including at least one (1) year of service with Continuum.
  • Primarily works within residential 12-hour and 24-hour programs. Provides direct support, skills training, and daily structure to clients under regular supervision.
  • Manages caseload within a structured program environment with ongoing supervision, mentorship, and team collaboration to support service delivery and professional development.
  • Implements established treatment and service plans developed in collaboration with supervisors and clinical staff.
  • Performs care coordination, documentation, crisis response, and service planning activities with regular supervisory support and consultation.
  • Develops foundational competency in client engagement, documentation, and community resource coordination.
