Overview

Acute Care Medicine

Acute Care Medicine nurses manage patients with a wide range of clinical issues, including significant blood dyscrasias, advanced endocrine pathology, heart and renal failure, gastrointestinal conditions, infectious diseases, asthma, cystic fibrosis, and numerous other conditions and diseases. Due to the patient acuity, new nurses receive advanced training to reach the highest level of functioning. Each unit is led by a clinical manager and a lead charge nurse, supported by an advanced practice nurse, case manager, social worker, physical therapist, dietitian, and pharmacist. This multidisciplinary team collaborates with physicians, nurses, and patient care technicians to provide quality patient care.

Role Purpose

Provides direct patient care activities including assessment, diagnosis, planning, implementation, and evaluation within the guidelines of the standards of nursing care.

Responsibilities

• Promotes patient and family-centered care in a healing environment.
• Educates patients and their families on how to manage their illness or injury, including post-treatment home care needs and medication administration.
• Participates in activities that promote patient safety, quality, and regulatory compliance.
• Participates in professional development.
• Develops, implements, and documents individual plans of care with defined goals in collaboration with other members of the interprofessional team and patient, family, or caregiver in accordance with the established guidelines and standards of nursing care.
• Proactively plans and ensures communication of the plan of care across the continuum of care.
• Uses critical nursing skills to assess and evaluate physical, psychosocial, and emotional needs according to standards of care.
• Assesses patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population-specific needs.

• Education: Nursing Diploma/Associate's Nursing

• Licenses & Certifications: RN

• Education: Bachelor's Degree Nursing
