Acute Care Clinical Pharmacist Specialist

About the role

Banner Children's Hospital at Desert in Mesa, Arizona is seeking a Clinical Pharmacist Specialist to support the Pediatric Oncology program. The role involves collaborating with the multidisciplinary oncology care team and staff pharmacists.

Responsibilities

• Participates in the optimal selection of drug therapy and designs a regimen to maximize therapeutic benefit and minimize toxicity.
• Ensures appropriate and cost-effective medication therapy.
• Evaluates disease state management, medication management, and adverse drug reactions.
• Adheres to clinical practices approved by Pharmacy and Therapeutics Committee/Clinical Consensus Group.
• Accurately and completely documents patient assessment data and clinical interventions.
• Provides detailed drug information and education to healthcare team, pharmacy, patients, and community.
• Reviews and evaluates medication orders for appropriateness and completeness with respect to patient data.
• Applies principles of continuous quality improvement consistent with job expectations.
• Participates in drug use evaluations and facility level medication safety oversight.
• Serves as a resource and mentor to other pharmacists, students, residents, and other health-care providers.

Requirements

• A Doctorate in Pharmacy degree and at minimum completion of a PGY1 residency program.
• Without residency training, a minimum of 3 years of health-system progressive clinical pharmacy experience (evaluating disease state & medication management).
• Board certification through the Board of Pharmaceutical Specialties (e.g. BCPS, BCOP).
• Arizona Pharmacy Licensure.

Qualifications

• Knowledge of medication management software, particularly pharmacy system software, Microsoft applications, and automated dispensing systems.
• Good organizational, problem-solving, and computer skills.
• Good written and verbal communication skills for interfacing with all levels of staff, physicians, and other contacts.

Skills

• Expert level working knowledge of principles, practices, and operations in assigned area of responsibility.
