About the role

Ten Broeck Center for Rehabilitation and Nursing is seeking a Full-Time Activity Leader for their Memory Care unit. This position is a Full-Time day shift with alternating weekends.

Responsibilities

  • Assists in providing recreational and other activities as per the individualized activity interests and multidisciplinary care plan.
  • Leads or facilitates various activity groups (religious services, physical and cognitive games, arts and crafts, gardening, grooming, etc.).
  • Communicates with residents with visual, speech, auditory, cognitive, physical, and emotional impairments.
  • Completes Activities section of MDS forms as scheduled.
  • Regularly assesses the social and emotional needs of the residents with the Director and sets appropriate goals for meeting those needs within the overall plan of care.
  • Helps in keeping activity materials organized.
  • Helps decorate the facility with seasonal decorations.
  • Provides input on monthly activity calendars and hangs them on appropriate units.
  • Works with the interdisciplinary team, attending various weekly care plan meetings.
  • Reports all cases of alleged violations involving mistreatment, neglect, or abuse, including injuries of unknown sources.
  • Affords assistance with transportation, if needed, to and from activities.
  • Affords assistance to residents to participate in individual activities of choice (setting up equipment, positioning assistance, providing sufficient lighting and space).
  • Affords information to residents about various activities available to them even when Activities staff are not available.
  • Affords assurance of safety of residents at all times.
  • Performs other duties as requested.

Qualifications

  • Bachelor's degree in a related field preferred.
  • Experience in a healthcare setting preferred.
  • Knowledge of memory care principles and practices.
  • Ability to communicate effectively with residents, families, and interdisciplinary team members.
  • Ability to organize and manage activities.
  • Ability to work independently and as part of a team.
