About the role
Ten Broeck Center for Rehabilitation and Nursing is seeking a Full-Time Activity Leader for their Memory Care unit. This position is a Full-Time day shift with alternating weekends.
Responsibilities
- Assists in providing recreational and other activities as per the individualized activity interests and multidisciplinary care plan.
- Leads or facilitates various activity groups (religious services, physical and cognitive games, arts and crafts, gardening, grooming, etc.).
- Communicates with residents with visual, speech, auditory, cognitive, physical, and emotional impairments.
- Completes Activities section of MDS forms as scheduled.
- Regularly assesses the social and emotional needs of the residents with the Director and sets appropriate goals for meeting those needs within the overall plan of care.
- Helps in keeping activity materials organized.
- Helps decorate the facility with seasonal decorations.
- Provides input on monthly activity calendars and hangs them on appropriate units.
- Works with the interdisciplinary team, attending various weekly care plan meetings.
- Reports all cases of alleged violations involving mistreatment, neglect, or abuse, including injuries of unknown sources.
- Affords assistance with transportation, if needed, to and from activities.
- Affords assistance to residents to participate in individual activities of choice (setting up equipment, positioning assistance, providing sufficient lighting and space).
- Affords information to residents about various activities available to them even when Activities staff are not available.
- Affords assurance of safety of residents at all times.
- Performs other duties as requested.
Qualifications
- Bachelor's degree in a related field preferred.
- Experience in a healthcare setting preferred.
- Knowledge of memory care principles and practices.
- Ability to communicate effectively with residents, families, and interdisciplinary team members.
- Ability to organize and manage activities.
- Ability to work independently and as part of a team.
