Activities Coordinator for Memory Care

Provide creative and interactive programming for 20 residents with varying stages of cognitive decline in our memory care community, in accordance with federal, state, local, and corporate standards. Design an on-going program of activities to meet the interests and physical, mental, and psychosocial needs of each resident based on comprehensive assessments.

Responsibilities

  • Plan and conduct individual, small, and large group activities.
  • Facilitate communication between employees, residents, families, support personnel, government agencies, and the public to meet resident and community needs.
  • Participate in community planning related to senior living services and resident/family needs.
  • Develop and maintain a monthly activity calendar and attendance records.
  • Implement corrective action plans for identified quality deficiencies as directed by the Health and Wellness Director or Activities Director.
  • Arrange transportation for resident outings and appointments.
  • Create a monthly activity schedule including outings, group activities, and in-room activities for bed-bound or isolated residents.
  • Encourage resident participation in self-initiated activities (hobbies, crafts, reading) and provide necessary materials, including Braille or audio books.
  • Assist with assessment documentation and quarterly progress notes as assigned.
  • Maintain a clean, orderly, and secure department.

Requirements

  • High school diploma or equivalent; one-year experience in a memory care community preferred.
  • Ability to read technical procedures, policy manuals, and present information effectively.
  • Basic mathematical skills (fractions, percentages, ratios, proportions).
  • Ability to solve practical problems and interpret varied instructions (written, oral, diagrams, schedules).

Physical Demands

  • Frequent standing, walking, reaching, pushing/pulling, talking, hearing, tasting/smelling, and lifting up to 50 pounds.
  • Occasional sitting, climbing, balancing, stooping, kneeling, crouching, or crawling.
  • Visual abilities including close, distance, color, peripheral, and depth perception with ability to adjust focus.

Work Environment

The noise level is usually low to moderate. Reasonable accommodations may be made for individuals with disabilities.

Pay

$22.00–$24.00 per hour, based on experience.

Schedule

Monday–Friday.
