Essential Duties
- Participate in planning and conducting of individual, small and large group activities.
- Aid in communication between employees, residents, families, support personnel, government agencies, and the public to ensure the needs and best interests of residents, community, and facility are met.
- Engage in community planning related to the interests of the facility and the services and needs of the resident and family.
- Contribute to the development of a monthly activity calendar and maintain attendance records.
- Support the Quality Assurance and Assessment Committee in developing and implementing corrective actions for identified quality deficiencies as requested by the Activities Director.
- Assist in discharge planning, creation, and implementation of activity care plans and resident assessments.
- Arrange transportation for residents when necessary, including resident outings, appointments, or discharges.
- Develop a monthly activity schedule for residents, which includes resident outings, regularly scheduled group activities, and in-room activities for bed-bound or isolated residents.
- Encourage residents to engage in self-initiated activities, including hobbies, crafts, and reading.
- Provide necessary materials, such as reading materials in Braille or audio books, as appropriate.
- Assist with assessment documentation and quarterly progress notes as assigned by the Activity Director.
- Maintain the Activity Department clean, orderly, and secure.
Qualifications
- High school diploma or equivalent.
- Preference for one year experience in a long-term care facility.
- Ability to read technical procedures and policy and procedure manuals.
- Effective presentation and response to questions from managers and employees.
- Application of concepts such as fractions, percentages, ratios, and proportions to practical situations.
- Solving practical problems.
- Interpreting a variety of instructions in written, oral, diagram, or schedule form.
