Activities Assistant HCC
Job Purpose
Aids in planning, implementing, and evaluating recreational, social, intellectual, emotional, and spiritual programs based on patient assessments and care plans, and as directed by the Supervisor.
Key Responsibilities

Conducts assessments and gathers information through interviews with residents, family members, legal representatives, and significant others to develop individualized recreation programs that cater to residents' functional levels, needs, and interests.

Updates resident recreation programs as necessary to align with individual needs and preferences while protecting and promoting residents' rights.

Collaborates with interdisciplinary team members to develop, implement, and evaluate the plan of care, attend care conferences, and develop activity interventions.

Communicates resident concerns and intervention responses to interdisciplinary team members and direct care staff.

Schedules recreational activities that appeal to all age groups, both men and women, and take place in various locations, including special seasonal events.

Posts a monthly Activities calendar.

Provides activities to residents confined to their rooms that reflect their lifelong interests, such as music, reading, visits with people of shared interests, and in-room projects.

Develops and implements structured activities for cognitively impaired residents, such as music events, gross motor games, or outdoor walks, and adapts activities to ensure all residents can participate.

Ensures all residents are informed about activities, staff encourages participation, and transportation is provided.

Records progress notes in the clinical record, including subjective findings, objective symptoms, observations of behavior, interventions provided to residents, and resident responses to interventions.

Audits chart entries for completeness and accuracy.

Performs quality assessment and assurance functions, including daily regulatory compliance rounds, observation of activities in progress, record reviews, and interviews with staff members, residents, families, and Administration.

Identifies and addresses issues in Recreation Services staff performance and implements corrective actions.

Develops and updates Recreation Services policies and procedures that align with the facility's philosophy, professional standards, and state/federal laws and regulations.

Communicates with the Nursing Home Administrator daily on resident care, personnel, and budget matters.

Develops a staffing plan and recommends to the Nursing Home Administrator the number and type of personnel needed to meet resident needs in compliance with state and federal laws and regulations.

Participates in the development of the department budget and provides relevant financial information to the Nursing Home Administrator regarding departmental financial needs and status.
Minimum Education Required
Requires a minimum of two (2) years of college education with courses in recreation, education, social work, other behavioral sciences, or clinical services.
Minimum Experience Required
Two (2) years of experience in a social or recreational program within the past five years, with one year of full-time employment in a patient/resident activities program in a healthcare setting.
