Essential Duties

  • Participate in planning and conducting of individual, small and large group activities.
  • Aid in communication between employees, residents, families, support personnel, government agencies, and the public to ensure the needs and best interests of residents, community, and facility are met.
  • Engage in community planning related to the interests of the facility and the services and needs of residents and families.
  • Develop and maintain a monthly activity calendar and attendance records.
  • Collaborate with the Quality Assurance and Assessment Committee to develop and implement corrective actions for identified quality deficiencies.
  • Assist in discharge planning, creation, and implementation of activity care plans and resident assessments.
  • Arrange transportation for residents, including outings, appointments, and discharges.
  • Develop and execute a monthly activity schedule for residents, including outings, group activities, and in-room activities for bed-bound or isolated residents.
  • Encourage residents to engage in self-initiated activities such as hobbies, crafts, and reading, and provide necessary materials like Braille or audio books.
  • Assist with assessment documentation and quarterly progress notes as assigned by the Activity Director.
  • Maintain cleanliness, orderliness, and security within the Activity Department.

Qualifications

  • High school diploma or equivalent.
  • Preference for one year experience in a long-term care facility.
  • Ability to read and comprehend technical procedures and policy manuals.
  • Effective communication and presentation skills.
  • Basic mathematical skills.
  • Solving practical problems and interpreting instructions.
