Activities Assistant and Transportation Specialist

About the role

We are currently seeking a Skilled Nursing Facility Activities Assistant / Transportation Specialist who is passionate about providing seniors with unparalleled short- and long-term care. This role offers opportunities for growth and development within a supportive and inclusive team environment.

Responsibilities

• Inspects the facility vehicle each day to ensure it is safe for resident transportation
• Affixes to ensure the facility vehicle’s maintenance is completed in accordance with manufacturer guidelines and as needed on a routine basis
• Safely and respectfully transports individuals to and from medical appointments, therapy sessions, and community outings
• Assists in lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Observes resident mood and behavior and reports any changes or concerns to the Activities Director
• Captures transportation needs and requests with other departments in a courteous and timely manner
• Reports changes in a resident’s condition to the resident’s nurse and supervisor
• Treats all residents with dignity and respect
• Promotes and protects all residents' rights
• Establishes a culture of compliance by adhering to all facility policies and procedures
• Complies with standards of business conduct, state/federal regulations and guidelines
• Maintains confidentiality of protected health information, including verbal, written, and electronic communications
• Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel
• Reports any retaliation or discrimination to the Administrator
• Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to the Administrator
• Protects residents from abuse and cooperates with all investigations
• Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy
• Participates in all life safety and emergency drills and trainings
• Fulfills responsibilities as assigned during implementation or activation of the facility’s emergency plan
• Reports work-related injuries and illnesses immediately to supervisor
• Follows established infection control policies and procedures
• Completes all assigned training and skills competency
• Maintains a clean driving record, without substantial violations, throughout the term of employment

Requirements

• A current and unencumbered driver's license in the state, Class B with passenger endorsement – preferred
• Certified Nursing Assistant in good standing with the state – preferred
• A high school graduate; Associate degree – preferred
• Crisis Prevention Intervention (CPI) – preferred
• Motivational interviewing – preferred
• Behavioral Therapy – preferred
• Able to work swing shifts and weekends
• Current CPR/BLS certification – preferred
• Trained in First Aid care and response
• Must not pose a threat to the health and safety of other individuals in the workplace
• Able to move throughout the workday
• Meets general health requirements according to facility policy, including medical and physical exams and checking immunity status to various infectious diseases
• Able to assist in evacuation of residents during emergency situations
• Able to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position
• May be subject to falls, burns from equipment, and/or odors throughout the day; encounter reactions from dust, tobacco smoke, disinfectants, automotive fuels and chemicals and other air contaminants
