The ACT RN position is available at one of our FQHCs in Phoenix. Terros Health is a leading integrated healthcare organization dedicated to providing primary care and specialized mental health and substance use treatment. Responsibilities

  • Provides direct clinical services to individuals, groups, and families in community-based settings.
  • Coaches members and families on behavioral and physical health conditions, self-management techniques.
  • Promotes treatment adherence and personal wellness within a recovery framework.
  • Captures and documents medication administration and observations in the Medication module.
  • Attends primary care and specialty appointments with the client.
  • Demonstrates critical thinking and takes initiative, working independently within the scope of practice.
  • Performs health evaluations, takes vitals, draws blood for lab tests, administers intramuscular medications, and coordinates care with other providers.
  • Oversight and monitoring of medication delivery, observation, and administration for ACT Team clients.
  • Participates as part of the member’s clinical team, taking full responsibility for integrated treatment services.
  • Coordinates care with ACT team members and other providers to ensure member’s needs are met and continued treatment in the community.
  • Works with informal support systems to provide clinical skills training.
  • Maintains compliance with ACT fidelity measures and utilizes assertive engagement mechanisms.
  • Routinely assesses member’s need for ACT services and gradually transitions eligible members to less intensive services.
  • Monitors member’s status during transition and reintroduces services as needed to ensure community stability.
  • Responsible for crisis services, coordinating hospital admissions and discharge planning.

Requirements

Must be an Arizona Licensed Registered Nurse.

Qualifications

  • Experience in community-based settings preferred.
  • Knowledge of ACT fidelity measures and assertive engagement mechanisms.
  • Ability to work independently and take initiative.

  • Excellent salary range: $74K-$84K depending on experience and education.
  • ACT Differential Pay: +$3/hour = $6240 annually.
