ACT / IDDT Case Manager (26-72)
About the role
The ACT/IDDT Case Manager is responsible for Person-Centered Planning, providing on-going linking, coordinating, and monitoring of services. This position is 40 hours a week, primarily Monday through Friday from 8:00 am to 5:00 pm, with some evenings and weekends required. The role involves in-person work at 1131 Ionia Ave NW, Grand Rapids, MI 49503, and frequent travel within the community.
Responsibilities
- Develop and implement Individual Plans of Service
- Provide on-going linking, coordinating, and monitoring of services
- Travel within the community on a daily basis
- Participate in rotating on-call shifts
Requirements
- Bachelor’s degree in human services related field
- One-year full-time experience working with persons with mental illness
- Valid driver’s license and reliable transportation
- Strong organizational skills and experience in person-centered planning
- Demonstrated understanding of severe mental illness and substance use disorders
- Effective communication and active listening skills
- Licensed BSW or Qualified Mental Health Professional (QMHP) with one-year full-time experience
- Knowledge of trauma-informed theories, principles, and practices
Qualifications
- Possession of a valid driver’s license and access to reliable transportation
- Lived experiences with mental illness/developmental disabilities/substance use disorders valued

Pay
The final job offer is based on several factors including internal equity, skills, education, experience, and credentials. Additional compensation includes a 10% premium pay.

This position is 40 hours a week, primarily Monday through Friday from 8:00 am to 5:00 pm, with some evenings and weekends required. Rotating on-call shifts are also involved.
