Join the Southwest Network Assertive Community Treatment (ACT) team as the housing specialist. This role focuses on providing housing support services following SAMHSA’s evidence-based practice of permanent supportive housing.
Responsibilities
- Conduct comprehensive assessments of housing, residential, and activities of daily living skills and needs.
- Lead service planning for adult daily living skills and housing support services.
- Coordinate, monitor, and provide direct services to members in a community setting.
- Participate in developing individualized service plans (ISPs) and provide or coordinate related services.
- Provide after-hours care, on-call coverage, and support to all members served by the ACT team.
Requirements
- Master’s or bachelor’s degree in behavioral health or a related field; OR a bachelor’s degree in a non-related field plus one year of related experience; OR an associate degree in any field plus one year of related experience; OR a high school diploma or equivalency certificate plus three years of related experience.
- Minimum one year of experience working with seriously mentally ill adults; ACT team or equivalent experience preferred.
- Minimum one year of experience working with behavioral health residential programs and/or housing-related services and resources.
- Valid Arizona driver’s license (must maintain).
- Must be at least 21 years of age.
