ACT Care Coordinator - QMHP
About the role
The Care Coordinator/Specialist provides psychosocial rehabilitative skills training to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions, utilizing a recovery based and trauma-informed perspective. Care Coordinators/Specialists operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill-set by forging therapeutic relationships with diverse community members. Interventions and services are typically provided in the community and require the use of a personal automobile and cell phone.
Responsibilities
- Deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need.
- Facilitate psychosocial rehabilitative skills training groups and/or utilize other evidence-based interventions or therapeutic modalities.
- Participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services.
- Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning.
- Work in partnership with racially and culturally diverse individuals to define and facilitate their desired life outcomes.
- May provide bilingual communication services and complete a language competency test.
- On-call responsibilities may be required dependent upon unit protocols.
Requirements
Education: Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred.
Licenses: Current and valid driver's license and a clean driving record.
Experience: Bilingual in Spanish. Experience in Human/Behavioral/Social Services.

Pay
$24.18- $25.39 an hour based on education, plus an additional 10% crisis stipend.
Schedule
Full-Time. Workdays: Monday - Friday (occasional on-call nights). Workhours: 8:00 AM - 5:00 PM (occasional on-call nights).
