About the role
This position performs care management for recipients assigned by the State of Alabama Medicaid Office. The role involves developing and acting on sound care plans in a timely manner to improve recipients' overall health, often through face-to-face meetings to understand their challenges. The position primarily serves home care and community-based populations needing care management and transitional services from inpatient facilities.
Responsibilities
- Perform care management for Medicaid recipients assigned by the State of Alabama Medicaid Office.
- Develop and implement timely, effective care plans to improve recipients' health outcomes.
- Conduct face-to-face meetings with recipients to assess challenges and needs.
- Provide transitional services for home care and community-based populations moving from inpatient facilities.
- Document electronic case records, including writing care plans and SMART goals.
- Analyze clinical situations and make informed, independent decisions.
- Collaborate with medical providers and community resource agencies.
- Travel as required for job-related duties.
Requirements
- Bachelor of Science in Social Work (BSW) or Master of Social Work (MSW).
- Active Alabama license in good standing as a Licensed Baccalaureate Social Worker (LBSW).
- Must live and work within the designated region or adjoining county.
- Strong verbal and written communication skills to engage recipients in their care plans.
- Ability to manage a heavy caseload and document electronic case records thoroughly.
- Experience analyzing clinical situations and exercising independent judgment.
- Experience working with medical providers and/or community resource agencies.
Qualifications
- Two years of complex pediatric experience preferred.
- Experience working in a home setting as a Case Manager or Care Coordinator preferred.
- Knowledge of Alabama Medicaid regulations and guidelines preferred.
- Knowledge of community-based agencies and resources is preferred.
