Responsibilities

• Support individuals to live as independently as possible
• Implement evidence-based treatment and care within a group living environment
• Provide guidance, support, and perform various levels of activities of daily living
• Ensure the health, safety, and well-being of individuals and maintain the physical site's cleanliness
• Affirmatively assist in developing individualized, person-centered treatment plans
• Aid in accessing and/or providing transportation for individuals
• Create opportunities for the development of valued roles and personal relationships in the community
• Review and complete all required documentation

Requirements

• High School Diploma, GED, or equivalent
• Valid Driver’s License and acceptable driving record
• Reliability, willingness to learn, and openness to new opportunities

Qualifications

Open Sky values skills, location, training, and population served in certain programs, offering an additional hourly differential for positions that qualify. This position requires a valid driver’s license and acceptable driving record.

Skills

Strong advocacy skills, ability to build relationships, and a commitment to community integration are essential.
