Responsibilities
- Support individuals to live as independently as possible
- Implement evidence-based treatment and care within a group living environment
- Provide guidance, support, and perform various levels of activities of daily living
- Ensure the health, safety, and well-being of individuals and maintain the physical site's cleanliness and safety
- Affirm the importance of community integration and advocate for individuals served
- Develop and implement individualized, person-centered treatment plans
- Aid in accessing and/or providing transportation for individuals
- Create opportunities for the development of valued roles and personal relationships in the community
- Review and complete all required documentation
Requirements
- High School Diploma, GED, or equivalent
- Valid Driver’s License and acceptable driving record
- Reliability, willingness to learn, and openness to new opportunities
