ACCS Outreach Counselor - South County Integrated Teams
About the role
Adult Community Clinical Services is a Department of Mental Health funded service that delivers evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals.
Responsibilities
- Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff.
- Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
- Provide side-by-side support and coaching to help individuals socialize.
- Conduct both 1:1 and group activities geared toward increasing Persons' readiness to engage in rehabilitation.
- Ensure individuals have adequate financial support through assisting with finding gainful employment or applying for entitlements.
- Participate in the On-Call rotation.
Requirements
- A High School Diploma, GED or equivalent, required.
- A valid Driver’s License, reliable and insured vehicle, and acceptable driving record, required.
- Reliability, willingness to learn, and being open to new opportunities.
Qualifications
The team at Open Sky is tight-knit, supportive, and multidisciplinary. Clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to individuals served. Outreach Counselors act as a liaison between Persons served, community agencies, and families to coordinate treatment including employment and education entities, emergency service providers, outpatient clinics and others. They are committed to cultivating a mutually respectful partnership, setting appropriate limits, and maintaining a helping role with individuals served and family members. Outreach Counselors encourage and empower individuals to see beyond and live beyond perceived limitations by providing them with the tools required for success!
