About the Role
The Account Manager will serve as the forward-facing representative of Summit Home Care and Hospice, establishing and maintaining relationships with target referral sources. Responsibilities include securing new referrals, developing marketing strategies, and presenting sales materials to grow business and meet sales targets.
Responsibilities
- Secure new referrals from potential partners, patients, customers, and physicians.
- Consistently meet with the Sales and leadership teams to discuss new business plans and updates.
- Deliver promotional, marketing, and service materials to referral sources.
- Maintain and update contact information for referral sources, including preferred methods of contact.
- Meet with case managers and social workers to discuss opportunities to serve their patients.
- Schedule and conduct meetings with physician offices, Assisted/Independent Living (AL/IL), and skilled nursing facilities to explain Summit’s services.
- Learn, understand, and communicate all materials related to Summit’s services for patients and families.
- Maintain and expand business relationships with industry partners.
- Adhere to professional standards, home care policies, and procedures.
- Resolve patient care issues by collaborating with referral sources, patients, caregivers, and family members.
- Facilitate coordination of services, resources, and events to support physicians, patients, caregivers, and the community.
- Participate in networking activities to promote Summit and increase closed referrals.
- Utilize internal and external EMRs and CRMs to procure referrals.
- Consistently meet or exceed sales goals as directed.
- Perform all other duties as assigned.
Requirements
- High school diploma or equivalent.
- At least 3 years of experience in sales and/or customer service with excellent communication skills.
- Experience in home health care, durable medical equipment, or skilled nursing facility preferred.
- Ability to build rapport with referral sources.
- Strong negotiation and organizational skills.
- Deadline and detail-oriented.
- Ability to work a schedule that may include weekends, holidays, and evenings.
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift up to 15 pounds at a time.

Pay
Annual Salary: $65,000 - $75,000 per year
Bonus Potential: $9,000 - $25,000 per year (based on sales goals)
