About the Role

Joining the Northern Sydney Local Health District (NSLHD) Aged Care Assessment Service (ACAS) will mean you will be part of a dynamic team with various sites providing Aged Care Assessment to NSLHD. Excellent education and clinical supervision is provided on an ongoing basis. The team welcomes applications from those with a strong interest and passion for working with older persons and who want to be part of an experienced, friendly team of Occupational Therapists (OTs), Physiotherapists (PTs), Social Workers (SWs), and Registered Nurses (RNs).

Applicants need to have a strong commitment to working to a high standard and within very tight time frames. People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

This role is based at Macquarie Hospital but will work across Northern Sydney LHD.

Responsibilities

• Undertake comprehensive assessments, formulate, and implement appropriate care plans for older people.
• Demonstrate discipline-specific clinical experience working in relevant aged care settings.
• Apply detailed knowledge of My Aged Care, related aged care programs, and services to assess and support both routine and complex aged care presentations.
• Use Microsoft Office Programs, electronic patient record systems, and e-learning sites effectively.
• Maintain a high standard of written and verbal communication, with precise, clinically sound, and timely documentation.

Requirements

• Relevant tertiary qualifications as per clinical discipline, with full AHPRA registration or eligibility for membership of the relevant Australian peak professional body for the discipline.
• Proven ability to work within tight time frames while maintaining high standards.
• This is a NSW Health Category A position which requires immunisation and screening for certain diseases as outlined in the NSW Health OASV Policy. You will be required to complete the OASV Undertaking/Declaration Form and TB Assessment Tool during the recruitment process.
