Schedule

• Full time
• Day Shift (Monday-Friday 8am to 5pm with every other weekend or call rotation)

Responsibilities

• Implements and monitors patient care plans to ensure effective treatment
• Monitors, records, and communicates patient condition as appropriate
• Serves as the primary coordinator of all disciplines for well-coordinated patient care
• Notes and carries out physician and nursing orders accurately and promptly
• Conducts holistic, individualized assessments of each patient considering age, physiological, and psychological status
• Administers medications and solutions as required
• Documents all phases of patient status during care
• Participates in the transfer of patient information to ensure continued quality care
• Leads and mentors staff to ensure optimal performance and competency in care delivery
• Maintains a safe and clean working environment by complying with policies, procedures, rules, and regulations
• Develops and maintains positive working relationships within the practice and other departments
• Assesses and coordinates patient's discharge planning needs with members of the healthcare team
• Other duties as assigned

Coverage Area: Visits - Cicero, Berwyn, Riverside, North Riverside, Stickney, Forest View, Lyons, McCook, Westchester, Hillside, Berkeley, Oak Park and/or Broadview

Requirements

• Associate's of Nursing [Required]
• Bachelor's of Nursing [Preferred]
• 1+ acute care clinical experience [Preferred]
• 1+ home health experience [Preferred]
• Registered Nurse (RN) [Required]
• Basic Life Support - CPR Cert (BLS) [Required]
• Driver's License (DL) [Required]

Pay

Pay Range $33.60 - $56.52

Sign On Bonus: For eligible applicants, up to $10,000
