Responsibilities

• Manage chronic disease for underserved geriatric patients (age 55+).
• Provide care in an outpatient PACE setting.
• No procedures required.

Pay

From $120.00 to $145.00 hourly (all‑inclusive, combining base pay with potential overtime, call, and holiday pay; actual rate depends on critical need, experience, qualifications, and other factors).

Schedule

• 40 hours per week.
• Approximately 12‑15 patients per day.
