About the role
The role involves delivering quality care to patients through teamwork, implementing policies and procedures for patient safety, and providing support for pain management and pharmacological measures. The position also requires mobilizing resources for complex cases, formulating and documenting discharge plans, and anticipating patients' needs.
Responsibilities
- Executes appropriate policies & procedures to assure patient safety.
- Supports pain management and pharmacological measures.
- Assists patient/family in identifying individualized comfort measures.
- Follows up on internal resources to determine the outcome of interventions.
- Mobilizes resources in complex cases to maximize patients' control and participation.
- Provides information and interpretation of the patient's condition and offers coping mechanisms.
- Affords patients and families information about their condition and coping mechanisms.
- Affords patients and families information about their condition and coping mechanisms.
- Formulates and documents a discharge plan that maximizes the patient's ability to continue with meaningful life activities.
- Integrates assessment and diagnostic information with intuition to foresee potential age-specific healthcare needs.
- Anticipates patients' needs and ensures handoff communication.
- Identifies issues related to patient education for medication management.
- Provides discharge planning which includes instructions on discharge medications.
- Uses discretionary judgment to modify patient care regimens.
- Facilitates appropriate responses from other health care team members.
- Develops a plan for adjusting staffing patterns as needed.
- Attends educational sessions to improve departmental needs.
- Participates in unit-based and hospital-wide committees.
- Participates in performance improvement activities.
- Prepares for Joint Commission readiness.
- Works towards achieving successful Patient Satisfaction scores.
- Participates in unit-level efforts to achieve successful Patient Satisfaction scores.
- Performs any other related duties as required or assigned.
Requirements
Education: Associate's degree or graduate from an accredited school of nursing.
Experience: None. Previous nursing experience preferred.
Other Credentials: AHA BLS - Healthcare Provider, Registered Nurse - NJ.
Knowledge and Skills: Strong problem-solving and decision-making skills, high interpersonal skills, excellent verbal and written communication skills, quick adjustment to change, considerable knowledge of nursing principles, practices, and standards.
Special Training: Knowledge of nursing skills, hospital practices, procedures, and standards.
