Responsibilities
- Delivers quality care through the nursing process of assessment, diagnosis, planning, implementation, and evaluation.
- Ensures patient safety by executing appropriate policies and procedures.
- Supports pain management and pharmacological measures, and assists patients in identifying individualized comfort measures.
- Mobilizes resources in complex cases to maximize patients' control and participation over their recovery.
- Provides information and interpretation of the patient's condition and offers coping mechanisms.
- Formulates and documents a discharge plan that maximizes the patient's ability to continue with meaningful life activities.
- Anticipates patients' needs and ensures handoff communication.
- Identifies and addresses issues related to patient education for medication management.
- Provides discharge planning which includes instructions on discharge medications.
- Uses discretionary judgment to modify patient care regimens and facilitates appropriate responses from other health care team members.
- Participates in unit-based and hospital-wide committees and performance improvement activities.
- Attends educational sessions to improve assessment abilities and prepares for Joint Commission readiness.
- Participates in efforts to achieve successful Patient Satisfaction scores and monitors HCAAPS measurements.
Requirements
- Education: Associate's degree or graduate from an accredited school of nursing.
- Experience: Registered Nurse experience required.
- Certifications: AHA BLS - Healthcare Provider, Registered Nurse – NJ or Registered Nurse – NLC multi-state, TNCC, NIH Stroke Scale Certification, ACLS, NRP, PALS.
- Special Training: Knowledge of nursing skills, hospital practices, procedures, and standards.
