About the role

The role involves delivering quality care to patients through teamwork, ensuring patient safety, and supporting pain management and pharmacological measures. The position also requires formulating and documenting a discharge plan, anticipating patients' needs, and facilitating appropriate responses from other health care team members.

Responsibilities

• Executes appropriate policies & procedures to assure patient safety.
• Supports pain management and pharmacological measures.
• Assists patient/family in identifying individualized comfort measures.
• Follows up on internal resources to determine the outcome of interventions.
• Mobilizes resources in complex cases to maximize patients' control and participation.
• Provides information and interpretation of the patient's condition and offers coping mechanisms.
• Affords patients and families information about their condition and offers coping mechanisms.
• Assesses patient's need for information and utilizes appropriate language and approach.
• Formulates and documents a discharge plan that maximizes the patient's ability to continue meaningful life activities.
• Foresees potential age-specific healthcare needs and identifies issues for patient education.
• Provides discharge planning, including instructions on discharge medications.
• Uses discretion to modify patient care regimens.
• Facilitates appropriate responses from other health care team members.
• Develops plans for adjusting staffing patterns as needed.
• Attends educational sessions to improve departmental needs.
• Participates in unit-based and hospital-wide committees.
• Engages in performance improvement activities.
• Prepares for Joint Commission readiness.
• Achieves successful Patient Satisfaction scores and monitors HCAAPS measurements.
• Performs any other related duties as required or assigned.

Requirements

• Education: Associate's degree or graduate from an accredited school of nursing.
• Experience: None preferred, but previous nursing experience is beneficial.
• Certifications: AHA BLS - Healthcare Provider, Registered Nurse - NJ, TNCC (within 1 year of hire), NIH Stroke Scale Certification (within 1 year of hire).
• Training: ACLS (within 6 months of hire), NRP (within 6 months of hire), PALS (within 6 months of hire).
• Knowledge and Skills: Strong problem-solving and decision-making skills, high interpersonal skills, excellent verbal and written communication, quick adjustment to change, knowledge of nursing skills, hospital practices, and procedures.

Qualifications

• Physical Demands: Frequent physical demands include pushing/pulling, twisting, bending, reaching forward, reaching overhead, squatting/kneeling/crawling, wrist position deviation, pinching/fine motor activities, keyboard use/repetitive motion, taste/smell, sitting, climbing (stairs/ladders), standing, walking, carrying objects, talking/hearing.
• Sensory Requirements: Very accurate vision (20/40 near/far), color discrimination, accurate depth perception, accurate hearing.
• Occupational Exposure Risks: Bloodborne pathogens, chemicals, airborne communicable diseases, dust/particulate matter.
