Summary

The Patient Care Assistant (PCA) II RN Student functions as a member of the patient care team, under the direction of the Registered Nurse (RN), and proficiently and consistently performs all aspects of the PCA role. The PCA II RN Student is responsible for supporting the RN with clinical tasks, including but not limited to blood glucose monitoring, completing EKGs, applying simple dressings, using aseptic technique and responding to medical emergency situations utilizing Heartsaver CPR and AED skills. In addition, the PCA II RN Student is responsible for responding to the needs of patients and family members and taking an active role in the efficient operation of the unit. The PCA II RN Student will also assure the practice of the Patient and Family Centered Care Model in an environment that exemplifies best practice.

Qualifications

• Must be a current nursing student who has completed Fundamentals of Nursing or equivalent med/surgical clinical rotation.
• High School Diploma or Equivalent required.
• Active AHA HeartSaver CPR and AED program certification required.

Responsibilities

• Provision of a Safe Environment: Anticipates and identifies patient safety risks, in cooperation with licensed nurse and takes appropriate action to reduce and/or eliminate the risks (maintains proper bed height, uses side rails and alarms appropriately, places call light within patient reach, provides frequent patient checks and assists patient with ambulation). Makes routine patient room rounds to address patient needs. Follows and maintains restraint policies under RN supervision. Provides 1:1 monitoring as a patient observer for patients at risk for injury. Follows established hospital infection control and safety procedures. Utilizes proper body mechanics.
• Organizational Ethics: Maintains and promotes an atmosphere reflecting the mission and values of the organization. Respects and supports culturally diverse staff and patient population. Delivers non-judgmental, non-discriminatory care sensitive to patient diversity. Respects confidentiality and privacy. Works cooperatively with all staff and resolves interpersonal conflicts. Dresses appropriately and wears visible ID badge.
• Management of Resources: Efficiently organizes time and prioritizes patient care needs. Works with other PCAs to facilitate care. Provides assistance to co-workers. Adapts to changing patient and unit needs (offering assistance, floating, adjusting assignments). Communicates with Charge Nurse/Nursing Supervisor to coordinate breaks. Maintains visibility and notifies charge/staff nurse when leaving the unit.
• Management of Information: Reviews and is knowledgeable of policies, procedures and work rules. Demonstrates understanding of emergency codes. Utilizes hospital E-mail and reviews meeting minutes/clinical updates. Participates in staff meetings and in-services. Maintains timely ongoing verbal communication with RN throughout shift regarding patient condition and delegated tasks. Notifies RN immediately of significant changes in patient condition or monitoring alarms. Accurately collects and records patient data (vital signs, intake/output, blood sugar, weights, restraint care) per hospital documentation standards.
• Collaborative Interdisciplinary: Maintains and fosters a teamwork approach to patient care.
• Patient/Family Education: Addresses patient comfort and information needs by providing informative non-clinical information, orienting patients/visitors, arranging rooms, distributing nourishments/mail/flowers, changing linens, refilling ice packs, running errands. In collaboration with the licensed nurse, reinforces patient education and provides basic care instruction (call light use, positioning, transferring, walking, toileting, feeding).
• Competency: Complies with established practice standards, policies, protocols, guidelines, and regulations (DPH, JCAHO, Boston fire codes, OSHA). Maintains and demonstrates compliance with competencies and care standards. Maintains competence in technical skills including: application of external monitors, use of CPM machine, performing EKGs, blood glucose monitoring, applying simple dressings, and Heartsaver CPR and AED skills. Assists in orientation of new PCAs and serves as resource to float staff.
• Quality Improvement: Participates in hospital-wide and unit-based SEAQ programs. Communicates quality patient care concerns to Nursing Director, Clinical Leader, and/or Nursing Supervisor.
• Patient Care: Completes patient care as assigned by the licensed nurse including:
  • Assisting with admission, discharge, and transfer of patients; preparing room and ensuring identification bands.
  • Transporting patients, belongings, equipment, and supplies.
  • Providing personal care: bathing, skin care, shaving, mouth care, grooming, perineal care, changing linens.
  • Providing basic skin care: back rubs, routine diabetic foot care, application of non-medicated ointments, elastic stockings, ace wraps, compression boots, turning/repositioning patients.
  • Assisting with elimination: urinary/condom catheter care, bedpan, commode, toileting schedules, emptying colostomy bags/urinals/drainage bags.
  • Carrying out planned care measures for comfort and pain relief (coordinating rest periods, minimizing noise, positioning).
  • Providing 1:1 monitoring as a patient observer and performing PCA II duties for assigned patient.
  • Performing simple dressing changes using aseptic technique.
  • Assisting nurses/health team members during treatments or procedures.
  • Setting up patients for meals, assisting with feeding, maintaining calorie counts, recording intake/output.
  • Taking vital signs, intake/output data, daily weights accurately and documenting completely.
  • Performing EKGs per policy and procedure.
  • Performing blood glucose monitoring per policy and procedure.
  • Documenting patient information timely and accurately on flow sheets.
  • Providing end of life care, including comfort measures and post mortem care.
  • Collecting, labeling, and sending specimens to the lab with appropriate requisitions.
  • Initiating patient belongings tracking form and securing belongings per policy, consulting RN if declined.
  • Responding promptly and courteously to call lights and patient requests, seeking assistance as needed.
  • Maintaining clean/orderly patient rooms and stocking supplies/equipment.
  • Making routine room rounds to un-clutter, clean, and organize patient rooms.
  • Checking inventories and restocking/replacing/assembling equipment and supplies.
  • Disposing of trash, soiled linens, medical waste, and disposable items appropriately.
  • Performing spot and spill cleaning as needed.
  • Performing checks to ensure equipment is operating properly and has current safety inspections before patient use.
  • Identifying broken/expired equipment, removing from service, notifying Charge RN/Staff Nurse.
  • Performing initial cleaning/decontamination of equipment/materials and arranging terminal cleaning.
  • Identifying emergency/crisis situations and initiating appropriate response actions, immediately communicating to RN and assisting during/after emergency.

Pay

$18.56 - $27.58/Hourly

Schedule

Per Diem: 16 shifts in 6 months. At least 1-2 weekend shifts a month. 8 hour shifts minimum. Weekend and Holiday Rotation. Day Shift. Rotating shift.
