About the Role

This is a full-time, day-shift position focused on providing advanced medical social work services, including psychosocial assessment, counseling, crisis intervention, and transition planning for patients and their families.

Responsibilities

• Assessment/Evaluation: Obtain social history information and identify patient needs by interviewing the patient/family, reviewing medical records, conferring with physicians and medical staff, and contacting other information sources as needed.
• Analysis and Evaluation: Reflect advanced competency in psychosocial assessment and diagnosis. Assess problems in terms of scope, consequences, and required interventions.
• Case Plan Development and Implementation: Develop individualized psychosocial case plans in collaboration with the healthcare team. Implement and monitor patient progress.
• Transition Planning: Coordinate discharge planning services, consult with healthcare providers and families to determine post-hospital needs, and provide information on available resources. Assist with referrals and offer support throughout the process.
• Crisis Intervention: Differentiate between emergency and non-emergency tasks, provide immediate supportive counseling, and assist with referrals to community resources when appropriate.
• Counseling/Support Services: Provide psychosocial counseling and treatment to restore patients and families to optimum social and physical health within their capacity.

Requirements

• Master’s degree in social work or a related field required.
• Two years of experience in medical social work preferred.
• Competence in providing advanced medical social work services, including psychosocial counseling for individuals and families.
• Skill in conducting clinical interviews and understanding medical terminology.
• Ability to communicate effectively with diverse backgrounds and ages.
• Knowledge of clinical interviewing techniques, social work principles, standards, and ethics.
• Basic understanding of Medicare/Medicaid regulations and child/dependent adult laws is desired.
• Valid Pennsylvania driver’s license and automobile liability insurance required.
